You can find the axis of symmetry in the middle of the roots, and then evaluate the expression at that x value. Fill in the standard form equation parts. VERTEX form and STANDARD form are the same thing.

Roots: 0 and 4.
MIDDLE is axis of symmetry: x=2.
The function at x=2: -0.5%282%29%282-4%29=-1%2A%28-2%29=2.

Function highlight%28f%28x%29=%28-0.5%29%28x-2%29%5E2%2B2%29.

Multiply and simplify the function definition if you want general form.
