Unfortunately, I don't think you have enough info. You either need 


a) The center and the lengths of the major and minor axes,


b) Four points that lie on the ellipse itself, or


c) The two focii and one point that lies on the ellipse (<a href="http://www.geogebra.org/cms/">Geogebra</a> provides you a way to determine the equation and it even graphs it).
