This is still the same line. Point-slope form can still be used, and it IS ALLOWED TO LOOK DIFFERENT because you are using values of a different point which is still ON the line. You know it is on the line because you used in when you found the slope in your first question.

y-5=%281%2F4%29%28x-6%29, and this should be equivalent to y-4=%281%2F4%29%28x-2%29.
Simplify each of them and find out!
