to use point-slope form, you have to be given one point and slope...
The standard point-slope equation looks like this:
y-y%5B1%5D=m%28x-x%5B1%5D%29 where m=slope, x%5B1%5D and y%5B1%5D are coordinates of given point
you are given point P (2, 5) and it means x%5B1%5D=2 and y%5B1%5D=5,
so, plug it in:
y-y%5B1%5D=m%28x-x%5B1%5D%29

y-5=m%28x-2%29
y=m%28x-2%29%2B5 ......this is all you can do without a slope
