Start with the formula for Slope: m=%28y-v%29%2F%28x-u%29 and expect you know the value of m, and you know the point (u,v). Multiply both members by (x-u):

m%28x-u%29=y-v
y-v=m%28x-u%29----------This may be taken as a generalized Point-Slope equation for a line.

Substitute your given values. For your #2, this is how you start:

Put this into either Slope-Intercept or Standard Form.
