Factor out 16 and 4 inside the radicals and you have:
sqrt%2816%28x%2B2%29%29+%2B+sqrt%284%28x%2B2%29%29
:
16 and 4 are both perfect squares so extract those from the radicals:
4sqrt%28x%2B2%29+%2B+2sqrt%28x%2B2%29
:
Both radical have like terms so just add them:
6sqrt%28x%2B2%29
