The vertex form is

y = 1(x-0)²+4
y = a(x-h)²+k, so the vertex is (h,k) = (0.4)

(-3,13), (-2,8), (-1,5), (0,4), (1,5), (2,8), (3,13)

y=2x^2+6x=12
You mistyped an = sign for either a + or a - sign

If you meant a + sign, y=2x²+6x+12
