Use the slope formula:

     f(x2) - f(x1)
m = ———————————————
        x2 - x1

where x1 = 3 and x2 = 0

     (9) - (4)      5       5
m = ——————————— = ———— = - ——— 
     (0) - (3)     -3       3 

Now substitute in the point slope formula:

   f(x) - f(x1) = m(x - x1)

                 5 
f(x) - f(3) = - ———(x - 3)
                 3

   f(x) - 4 = - ———(x - 3)

Distribute to remove the parentheses:
