the -2 and 2 were given I guessed on output but not sure how to write a formula off of this information

Your guesses are all spot on. You are given two ordered pairs, (1, -2) and (6, 2). Now all you need is the two-point form of an equation of a line:

where and are the coordinates of the given points.

Just plug in the numbers, do the arithmetic and simplify.
