You cannot write the equation of a line. You can write as many forms of a possible infinite number of forms of an equation whose solution set is the set of points that comprise a line through a given point and with a given slope as you like.
Use the point-slope form of an equation of a line:
where are the coordinates of the given point and is the given slope.
Then you can put your answer into whatever form(s) you like.
