Use the two-point form of an equation of a straight line:
where and are the coordinates of the given points.
Substitute your values, do the arithmetic, and then solve the equation for in terms of everything else. This means rearrange your equation so that is all by itself in the LHS and everything else is in the RHS. When you are done, your equation should have the form which is the slope-intercept form.
Note: It doesn't matter which of the points you decide is and which you decide is so long as you remain consistent throughout a given problem.
