Ignore the part. They are only trying to illustrate the slope-intercept form pattern follwed by your function definition.

And if you decided that 15 per year is the increase and your base population is 2000 in year 0, then your specific form of the function is correct.

The assignment gave you a set of input values, that is a set of values that, one at a time, need to be substituted for in your function. As you substitute each input value, perform the arithmetic to calculate the value of corresponding to that input value. Your answer should be another set whose elements have a one-to-one correspondence with the given set. Alternatively you could express your answer as a set of ordered pairs . Either way your answer set must have 5 elements to correspond to the 5 elements of the given set.
