If 94 students answered the first question correctly and 54 answered both questions correctly, then 94 minus 54 equals 40 students answered ONLY question 1 correctly. Likewise 69 minus 54 equals 15 students who answered ONLY question 2 correctly.

40 who only answered #1 correctly, plus 54 who answered both correctly, plus 15 who only answered #2 correctly is 109 students who got at least something right. Then 137 minus 109 leaves 28 students who did not answer either of the two questions correctly.
