the general equation is ___ y - k = a(x - h)^2

y - 4 = a(x - 5)^2

substituting to find a ___ 0 - 4 = a(1 - 5)^2 ___ -4 = 16a ___ -1/4 = a

so the equation is ___ y - 4 = (-1/4)(x - 5)^2
