The slope is given by %28y%5B1%5D-y%5B2%5D%29%2F%28x%5B1%5D-x%5B2%5D%29. Just decide which point will be 1 and which will be 2 (it doesn't matter), and plug in the numbers. If (2,5) is point 1, then x%5B1%5D=2, and y%5B1%5D=5.

Given two points you need to use the two point form of the equation of a line:

y-y%5B1%5D=%28%28y%5B1%5D-y%5B2%5D%29%2F%28x%5B1%5D-x%5B2%5D%29%29%28x-x%5B1%5D%29

Again, just plug in the numbers and do the arithmetic

y-5=%28%285-8%29%2F%282-4%29%29%28x-2%29
y-5=%28%28-3%29%2F%28-2%29%29%28x-2%29
y-5=%283%2F2%29%28x-2%29
2y-10=3%28x-2%29
2y-10=3x-6
-3x%2B2y=-6%2B10
3x-2y=-4 is the equation in standard (ax%2Bby=c form.
