You know you want a form, y=mx+b, and you will start with something like y-k=m(x-h); this will only require a small number of algebra steps.

Slope intercept form in general is y=mx%2Bb. Point slope form in general is y-y_1=m%28x-x_1%29 given the slope m and a point (x_1, y_1). You'll want to distribute the slope m.

y-y_1=mx-mx_1
Isolate the y term by adding y_1 to both sides.

y=mx-mx_1%2By_1. And we're done. You can convert backwards by doing the steps backwards, but inverting the operations.
