Presumably you would like to solve for y?
sqrt%28y%2B12%29+=+5 First, you square both sides to undo the square root.
y%2B12+=+5%5E2
y%2B12+=+25 Now subtract 12 from both sides.
y+=+13...there!

Square both sides to get:
y+12 = 25
y = 13
