The x intercepts you can find from solving the equation in its currently given standard form, by just setting y=0 and then solve the equation for x.

The y intercept you find by just setting x=0 and evaluating.

The general form asked, you obtain simply through "simplifying", carrying through the squaring and any other additions or subtractions.
