I don't know what "slop intercept form" is. If you perchance meant to say "slope-intercept form" then first use the two-point form:
where and are the coordinates of the given points.
Plug in your numbers, then do the indicated arithmetic. Then solve for in terms of everything else. You will end up with an equation that looks like where is the slope and is the -coordinate of the -intercept.
