This is actually very simple! The Point-Slope form is y+-+y%5B1%5D+=+m%28x+-+x%5B1%5D%29 where the +x%5B1%5D+ and +y%5B1%5D+ represent the coordinates of any point on the line and "m" represents the slope.

So all you have to do is replace +x%5B1%5D+, +y%5B1%5D+ and "m" with the numbers you have been given:
y+-+%28-6%29+=+%283%2F5%29%28x+-+4%29
This is Point-Slope form. Although most people would simplify the left side by changing the subtraction of -6 to adding positive 6:
y+%2B+6+=+%283%2F5%29%28x+-+4%29
And we're done!
