Simply:

Here's how you'd use it....

Say you have two points, and you want to find the slope between them...
(6,2) & (3,7)
Well, I'd observe that x1 = 6, and x2 = 3
I'd also take in that y1 = 2 and y2 = 7
Using the slope formula, we can find the slope between those two points
+m+=+%282-7%29%2F%286-3%29+
+m+=+%28-5%29%2F%282%29+
+m+=+-5%2F2+
The slope decreases 5 units and goes to the right 2 units
