(1) Horizontal asymptote y=34 means c=34.
y+=+ba%5E%28-x%29%2B34

(2) y-intercept 659 gives us the value of b:
659+=+b%28a%5E0%29%2B34
b+=+625

(3) Use the given point (2,134) to find a:
134+=+625a%5E%28-2%29%2B34
100+=+625a%5E%28-2%29
a%5E2+=+625%2F100
