The equation of an ellipse with major axis parallel to 
the x-axis

is:

%28x-h%29%5E2%2Fa%5E2%2B%28y-k%29%5E2%2Fb%5E2=1

where the center is (h,k)
where a = one-half the major axis
and b = one-half the minor-axis.

Now all you have to do is substitute for h, k, a and b.
