Remember slope? Vertical Change Divided by Horizontal Change.

m for slope.
Points, (x,y) and (u,v). x and y might or might not be known.
m=%28y-v%29%2F%28x-u%29, formula for slope.
m%28x-u%29=y-v
highlight_green%28y-v=m%28x-u%29%29, point-slope for for equation for a line.

Start with that form and substitute your given values.
