Now use the point slope form with either point,
y-y%5B1%5D=m%28x-x%5B1%5D%29
y-12=%283%2F13%29%28x-6%29
Now convert to slope-intercept form.
y=mx%2Bb
y-12=%283%2F13%29x-%2818%2F13%29
y=%283%2F13%29x-%2818%2F13%29%2B%28156%2F13%29
