Let's do the slope first.
Points (0,1) and (4,3) are on the line.
Calculate the slope using those two points.
m=%28y2-y1%29%2F%28x2-x1%29=%283-1%29%2F%284-0%29=2%2F4=1%2F2
Now use the point-slope form of a line,
y-yp=m%28x-xp%29
y-1=%281%2F2%29%28x-0%29
y=%281%2F2%29x%2B1

Pick two points. This graph includes a point that is the y intercept. So let's use that.
y+=+mx+%2B+b where m is the slope and b is the y intecept
Use points (0,1) and (2,2)
The y intercept is 1 (as given by the point (0,1)). So b = 1
Now let's find the slope
m = (y2 - y1)/(x2-x1)
m = (2 -1)/(2-0) = 1/2
The equation for the line is y+=+%281%2F2%29x+%2B+1
and the slope is 1/2
