d. Write the equation of the line in slope-intercept form.
e. Fill in the missing coordinates of the points (x, -3) and (-2, y).
I know how to solve all the problems except e. I just put all the problems here so whoever reads this can see what I've done so far.

y=mx+b, this form.

What is y, for (-2,y)?
y=m%28-2%29%2Bb
and you can evaluate according to YOUR equation.

What is x, for (x,-3)?
Solve for x in terms of y.
y-b=mx
mx=y-b
x=%28y-b%29%2Fm
and then
x=%28-3-b%29%2Fm
