You made a simple sign error.
When you expanded the numerator after putting everything on the left and applying the LCD, you should have had:

4x%5E2%2B16x-4x-16-%284x%5E2-1%29 in your numerator

Collecting terms you should have ended up with
12x-15 rather than 12x-17 because the minus sign distributes across the parentheses around the last term.

Your final equation should have been:

%2812x-15%29%2F%28%28x%2B4%29%282x%2B1%29%29=0 and then solve for the zeros of the numerator polynomial.

I'm not sure where you picked up the minus sign on your answer, but that isn't correct either.

So: x=15%2F12

%2815%2F3-12%2F3%29%2F%2815%2F6%2B6%2F6%29=1%2F%2821%2F6%29=6%2F21=2%2F7
