(h,k) = (0,4) so plug those in:

(2)   %28x-0%29%5E2+%2B+%28y-4%29%5E2=r%5E2

Now all we need is r2

So we plug in (x,y) = (1,4) in (2)

      %281-0%29%5E2+%2B+%284-4%29%5E2=r%5E2
      1%5E2+%2B+0%5E2=r%5E2
      1%2B0=r%5E2
      1=r%5E2

So plug in 1 for r2 in (2) and you have
the answer:
