begin by finding the slope m
P (0,0) and
Q (4,4) m = -4/-4 = 1
using the point-slope
y - 0 = 1(x-0)
y = x

2 points are all you need to determine a straight line
This is the formula:
+%28+y+-+0+%29+%2F+%28+x+-+0+%29+=+%28+4+-+0+%29+%2F+%28+4+-+0+%29+
+y%2Fx+=+4%2F4+
+y%2Fx+=+1+
+y+=+x+
This is a line with slope = +1 and y-intercept at the origin
The formula is just
( change in y ) / ( change in x ) = ( change in y ) / ( change in x )
