for a 18 month old weighing 28 pounds?

 1 pound  = 0.453592 kilograms.

28 pounds = 28*0.453592 kilograms = 12.701 kilograms.  // forgive me physicists for using 
                                                       // the kilogram as a measure of weight


To determine the dosage, multiply 5 mg per kilogram by 12.7 kilograms.


The answer is 63.5 mg.
