From the text book I cannot figure our the formula. I've tried putting in numbers for x and solving for f. I've also tried using the quadratic equation function on this site. A detailed explanation would be appreciated.

this equation is in "vertex" form __ y=a(x-h)^2+k __ where (h,k) is the vertex
__ the axis (of symmetry) passes through the vertex and has the equation x=h
vertex __ (-4,0)
axis __ x=-4
x can be any value so the domain is all real numbers
because of the square function, y can't be less than 0; so the range is all real numbers >= 0
