Well since we are given the slope and the y intercept the easiest way to write the equation is in slope intercept form. Which is:
y = mx + b
In slope intercept form the m is the slope and b is the y intercept
so just plug in the given values for each.

if you need to write the equation in standard form which is:
Ax + By = C
you can just convert the slope intercept form to standard form
to do this first you need to get rid of any fractions by multiplying through by the LCD (lowest common denominator)

multiply through by 6

next move the x term over
