y-y%5B1%5D=m%28x-x%5B1%5D%29

Where

x%5B1%5D is the x-coordinate of the given ordered pair,

y%5B1%5D is the y-coordinate of the given ordered pair, and

m is the given slope.

Just put your numbers where indicated and you are good to go. You may have to re-arrange your equation depending on whether you are asked to present it in standard form ax%2Bby=c or slope-intercept form y=mx%2Bb.
