find amplitude,period,frequency,phase shift, and vertical shift. than graph two periods of the functions for y=sin(x+5pi/6)+4

Form of equation for sin function: y=Asin(Bx-C)+D, A=amplitude, period=2pi/B, phase shift=C/B, D=vertical shift.
For given sin function:
Amp;itude=1
B=1
period=2pi/B=2pi
frequency=1/period=1/2pi
C=5pi/6
phase shift=C/B=5pi/6 (shift left)
D=vertical shift 4 units up
y-intercept:
set x=0
y=sin(x+5pi/6)+4=sin(0+5pi/6+4=1/2+4=4 1/2
