square both terms

(x+4)+4(sqrt(x+4)+16=x+20
x+20 +4(sqrt(x+4)=x+20
(-x -20)
4(sqrt(x+4))=0
/4
sqrt(x+4)=0

x+4 = 0
-4
x=-4
