Presumably you would like to solve for y?
First, you square both sides to undo the square root.
Now subtract 12 from both sides.
...there!

What is the square root of y+12=5
--------------------
sqrt(y+12) = 5
Square both sides to get:
y+12 = 25
y = 13
----------
Check that answer in the original equaltion:
sqrt(13+12)=5
5 = 5
