Rewrite each term with the lcd as its denominator:

[(u+x)u/lcd] + [(5u+x)*x/lcd]

Combine the numerators over the lcd:
[u^2+ux + 5ux+x^2]/lcd

= [u^2+6ux+x^2]/u^2x^2
