a. Population. This is easily countable.
b. A sample--there is no way it can be tested on everybody of concern.
c. Population--you could sample, but I count things up to 600 easily.
d. Sample--there are thousands of doctors, and it's going to be difficult to count all of them. Besides, the question asks for an estimate, and that means "SAMPLE."
