is the difference of two squares, so the factorization is (x\ -\ 4))
Hence
Apply the LCD:
\left(\frac{x}{x\ -\ 4}\right)\ =\ \frac{11}{x^2\ -\ 16}\ +\ 2\left(\frac{x^2\ -\ 16}{x^2\ -\ 16}\right))
