The first thing you need to do is add the two rational expressions. Just like adding any sort of fractions, you need a common denominator. Since neither of the denominators have any factors in common, the LCD is simply the product of the two denominators.
\left(\frac{x\ +\ 4}{x\ +\ 4}\right)\ +\ \left(\frac{4}{x\ +\ 4}\right)\left(\frac{x\ +\ 2}{x\ +\ 2}\right)\ =\ 3)
\ +\ 4(x\ +\ 2)}{x^2\ +\ 6x\ +\ 8}\ =\ 3)
Simplify the numerator and multiply both sides by the denominator:
