I just gave a response to this same question. Use your algebra skill as I said there.
Note, starting with point-slope form, y-v=m(x-u) for some point (u,v) on the line, transform into whichever other form you want.
----slope-intercept form, although the way you might handle the values depends on what they are and/or their signs.
What you do for standard form depends on any denominator of m, since you want to try to get integer coefficients for x and y if possible.
