The question is to write THE equation not an equation. There is only one equation and that is
(1) y = m*x + b or
(2) y = (1/2)*x + 3

(1) y = m*x + b or
(2) y = (1/2)*x + 3
is one solution.
I beg to differ with the other tutor.
There are an infinite number of equations with m=1/2 and b=3
2y=x+6
4y=2x+12
