slope intercept form equation is when given these points : (-4 , 1) , (0 , 16)

y = mx + b
------
slope = (16-1)/(0--4) = 15/4 =
-----
y- intercept: f(0) = 16
----------------------
Equation:
y = (15/4)x + 16
