The vertex form is

y = 1(x-0)˛+4
y = a(x-h)˛+k, so the vertex is (h,k) = (0.4)

(-3,13), (-2,8), (-1,5), (0,4), (1,5), (2,8), (3,13)

and for the equation

y=2x^2+6x=12
You mistyped an = sign for either a + or a - sign

If you meant a + sign, y=2x˛+6x+12

(-4,20), (-3,12), (-2,8), (-1.5,7.5), (-1,-8), (0,12), (1,20)

If you meant a - sign, y=2x˛+6x-12
