Start with point slope form: y - y1 = m(x - x1)
In this problem
x1 = -3
y1 = 5
m = 2
:
y - 5 = 2(x - (-3))
y - 5 = 2(x + 3
y - 5 = 2x + 6
y = 2x + 6 + 5
y = 2x + 11; is the slope/intercept form
