Write the equation of the line with slope of 5 and a y-intercept of (0,4)
A) y = 5x + 4 B) 5x = 4y C) 4x = 5y D) y = 4x + 5

Use the form y = mx + b

where m = the slope and b = the y-coordinate of the y-intercept (0, b)

So in your problem m = 5 and b = 4, so substitute 5 for m and 4 for b in

y = mx + b and you get

y = 5x + 4

which is choice A).  What could be easier?
