First you have to find the slope of the two points. The equation for this is:
y2-y1
-----
x2-x1

If you plug in the points you gave me, it would look like this:
1-6
----
-3-5

8

Then, you plug this and one of your points in with the equation y-y1=m(x-x1) like this:
y-6 = 5/8 (x-5)
