1st find the slope, m
m = diffy/diffx = (-1/3 - 3)/(1/6 - 5) = (-10/3)/(-29/6)
m = 20/29

Use y = mx + b and either point to find b:
3 = (20/29)*(5/6) + b
b = 37/87

y = (20/29)x + 37/87 slope-intercept form
60x - 87y = -37 standard form
