Complete the square on the right side, as follows:
y = (x^2-4x+4)-5-4
You need the +4 and the -4 to keep the expression unchanged.
y = (x+2)^2 -9
So, comparing this to the form y = a(x-k)^2+h
a=1,k=-2,h=-9
